I agree it is culture but it goes further. Someone is in a leadership role in every one of these situations and they are implicitly endorsing these behaviors. They have the power to shift the culture or try to correct it. Rather that culture being an emergent etherial thing, I prefer to look at who is protecting the culture as they are also the people who are responsible for its problems.
